Please print, complete and sign the Patient Information, Office Policy Regarding Insurance and Payment, Private Practices Acknowledgement and Release forms. Bring these with you to your first visit. We can discuss any questions.

Read the Notice of Client Privacy Practices and Treatment Philosohpy documents. Again, if you have any questions, I am happy to answer them for you at the office.
